Choices and Solutions currently offer the following services: 

Utilization Review:  

This service includes a comprehensive analysis of a certain percentage of the agency’s patient charts. Each chart consists of one or more certification periods. We analyze the certification period that falls within whichever quarter we working on. Within each certification is a plan of care, orders, medication profile, discipline notes, and as needed, communication notes. 

Quarterly Reports 

This is service includes an analysis of different aspects of the company such as infection control, incident reports, and complaints. The analysis is done within the quarter, and expands to include a yearly analysis. 

Plan of Action/Corrections: 

This service includes a plan to correct deficiencies found by the state, or the local accreditation body, as well as by our office. The plan is designed to help the agency be, and remain complaint. 

Staff Training:  

This service includes a comprehensive training of our clients’ staff. C&S offers training for administrators, directors, office, and field staff.  

Agency Evaluation:  

This service includes an evaluation of agency operations. C&S offers an overall evaluation, or a particular evaluation of certain departments. 

Cost Reports:  

This service is performed by C&S accounting staff that has over 20 years of home health experience.
