Quality Assurance Performance Improvement  (QAPI) Reports

This is comprehensive review and analysis based on CMS Guidelines and State Regulations that includes the following:

  • Infection Control

  • Hospitalizations

  • Incidents

  • Clinical Record Reviews

  • Personnel Files

  • Complaints

Utilization Review Reports                

This is active & discharge chart reviews of a percentage of your total census based on CMS Guidelines and State Regulations. A comprehensive review and analysis will be completed that includes the following:

  • Orders – Is the plan of care present and signed by clinician and physician? Is there an order for each change to the plan of care that is signed by clinician and physician?

  • Clinical Notes (nursing, therapy, etc) - Is there a completed OASIS that explains the reason (s) for home health, and does the clinician notes support those reason(s).

  • Coordination of Care (communications, referrals, etc) – Is the physician aware of abnormal findings? Is there effective communication among disciplines?

Staff Training

Have you hired a new Director of Nursing or a field nurse that is new to home health? We can help train your staff! We assess the overall agency’s operation and the current knowledge of the employee, and then build upon that employee’s knowledge, incorporating it into the agency’s operation. Cost Reports

This is a report that CMS requires from each home health agency.  We prepare the report and give the home health agency two (2) copies with 2 discs.

Your cost report will be effectively and efficiently prepared by over 20 years of home health accounting experience.

Agency Evaluation

This is an overall and comprehensive evaluation of the agency’s operation. The report includes evaluations of Professional Advisory Meetings, Governing Body Meetings, Emergency Preparedness, as well Human Resources and QAPI.

Plan of Corrections

This is a Plan to correct the deficiencies the home health agency received as a result of a survey from the state.

We also work with deficiencies received resulting from

Joint Commission and CHAPS surveys.
